*ESSENTIAL JOB DUTIES*
* Provides each resident with professional nursing care, assessments and evaluations.
* Responsible for the management of nursing activities performed by Neighborhood/Household (NH/HH) staff ensuring that the highest degree of quality care is maintained at all times
* Works with a team of cross-trained staff to coordinate the functions of the NH/HH
* Has 24 hour responsibility for clinical functions of the NH/HH

*Qualifications:*
* Licensed Practical Nurse is required
* Minimum of one (1) year experience in long term care.
* Ability to effectively communicate, direct and delegate tasks.
* Three (3) years MDS experience preferred
