Job Summary:

We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care, conducting assessments, and developing care plans to ensure optimal patient outcomes.

Duties:

- Conduct comprehensive patient assessments
- Develop individualized care plans in collaboration with the healthcare team
- Monitor and evaluate patient progress
- Coordinate with healthcare providers to ensure continuity of care
- Provide patient education on treatment plans and self-care strategies
- Utilize utilization management techniques to optimize patient care
- Maintain accurate and up-to-date patient records

Requirements:

- Valid RN license
- Proven experience in utilization management and case management
- Strong knowledge of physiology, vital signs, and hospital medicine
- Experience working in various healthcare settings such as clinics, hospitals, critical care units, and behavioral health facilities
- Ability to work effectively with patients across different age groups including geriatrics

Pay: $65.00 - $70.00 per hour
