  • Must have at least 2 years minimum discharge planning experience.
  • Strong acute care hospital experience required.
  • Candidates must have Cerner system experience.
  • Requires at least 2 current references covering 24 months out of the last 36 months, with one reference from the most recent job.
  • Schedule Information
  • Work from Monday to Friday, 8:00 AM - 4:30 PM.
  • Expected to work every 4th or 5th weekend and holidays as needed.
  • Additional Details
  • Orientation is every other Monday.
  • Start date: ASAP.
  • Expected hours are 40 per week.
  • Documentation must be in a week prior - set orientation dates.
  • It's important to disclose if the candidate has ever worked at this system before submission.
