*Responsibilities:*

· Conduct thorough assessments of clients in their homes, focusing on mobility, strength, flexibility, and overall physical function.

· Develop personalized treatment plans based on home assessments, considering the unique needs and living environments of clients.

· Implement a range of therapeutic interventions, exercises, and modalities to improve clients' mobility, strength, and functional abilities within the comfort of their homes.

· Provides supervision to Physical Therapist Assistants including case conferencing, co-signing treatment notes, and onsite client supervisory visits.

· Provide client education, ensuring a clear understanding of conditions and treatment plans, and offering guidance on home exercises for ongoing progress.

· Regularly reassess clients and modify treatment plans as needed to achieve optimal outcomes in a home-based setting.

· Utilize effective pain management techniques to enhance clients' overall well-being.

· Foster clear communication with clients, their families, and healthcare team members to provide personalized and effective home-based care.

*Requirements:*

· Active New Jersey Physical Therapist License.

· 2+ years preferred but new graduates are encouraged to apply.

· Valid New Jersey Driver’s License.

· Demonstrate unwavering professionalism with clients, team members, and families.

· Exhibit strong interpersonal, verbal and written communication skills.

· Showcase the ability to adeptly navigate as needed.

· Must travel throughout Union County to service clients.
