Springpoint at Home is seeking an experienced Registered Nurse Home Care Supervisor to join our team
**This position is Full-Time, M-F with weekend rotations based out of Monroe, NJ

Springpoint at Home offers home care and care management services to residents of our Life Plan Communities and to individuals that live in their private home. At Springpoint, our RN Home Care Supervisor plans, coordinates, manages, and evaluates the activities of the professional nursing and CHHA team to ensure the delivery of high-quality home health care services to clients.
Responsibilities:
• Responsible for the supervision, instruction, support, and coaching of CHHA staff on providing quality assured home care services in an organized and cost-efficient manner.
• In the absence of an Intake Schedule Coordinator, performs and/or delegates associated tasks.
• Reviews and authorizes payroll entries and maintains timeline to assure completion.
• Making 30-day phone calls to monitor client’s health, discuss performance of aides, and update care plans (or more frequently as needed).
• Make 60-day client visits to perform re-assessments and observe/supervise home health aides.
• Reviews client records for appropriate documentation at admission, recertification, and discharge.
• Conducts regularly scheduled multi-disciplinary client/case review conferences and is ongoing as needed.
• Monitors and ensures coordination and implementation of established patient care plans, discharge planning, and related activities.
• Supervises and ensures the timeliness, quality, and compliance standards of documentation of assigned staff.
• Collaborate with the scheduling team regarding staff utilization, retention, engagement, and schedule fulfillment.
• Assists staff in orientation and staff development/in-service programs and monitor ongoing continuing education by CHHA staff.

Education and Experience:
• Highschool Diploma or equivalent required
• Bachelor’s degree in nursing or related field preferred
• Valid NJ RN License
• CPR Certified
• Minimum 2 years’ experience as a Community Health Nurse or RN working in a Certified Home Health Agency
