Job Overview Shriners Children's, the premier pediatric burn, orthopaedic, spinal cord injury, cleft lip and palate, and pediatric subspecialties medical center, has an opportunity for a RN Care Manager to join our team. Responsibilities The RN Care Manager is responsible for providing assessments, planning, interventions and follow-up for patients and family members at Shriners Children's across the continuum of care. Through this function, by joining the team, you'll have the opportunity to make an impact on our patient's experience. You'll help to provide continuity of care for positive outcomes in our family centered environment. Qualifications BSN required, MSN preferred. Required experience: * 5+ years of clinical experience in Pediatrics, Spinal Coord Injury and/or Orthopaedics required. * 3+ years nursing required. * RN Illinois Licensure required. * 1+ years Care Coordination/Case Management experience in Pediatrics is preferred. * Case Management Certification (CMC, ACM or ANCC) required within 12 months of hire
