Responsibilities

Enhabit is seeking a Registered Nurse (RN) to join our team of hospice professionals. Our Hospice RNs work in private homes, residential care facilities, nursing centers, and other hospice care environments and must be capable of compassionate communication with patients and families, have keen observation skills, high ethical standards, and knowledge of when to alert doctors and others about changes in patients' conditions.   

 

Responsibilities include:

  • Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs
  • Develop and implement individualized Care Plans based on patient assessments in the EMR system within 48 hours
  • Administer medications and treatments as prescribed by physicians plus call medication into pharmacy
  • Monitor patients' condition and document changes in their health status
  • Collaborate with interdisciplinary teams to coordinate patient care
  • Provide education and support to patients and their families on end-of-life care options
  • Assist with pain management and symptom control
  • Offer emotional support to patients and their loved ones
  • Attend IDG meeting and trainings
  • Must input and complete visit notes timely
  • Perform services in accordance with the physician’s orders and the established plan of care
  • Ensure delivery of care is compliant with the federal and state regulations.


Qualifications
  • Must be Registered Nurse (RN) licensed to practice in the state
  • Minimum of 1 year of hospice experience
  • Must have basic demonstrated technology skills, including operation of a mobile device.
  • Medicare home health or hospice experience is preferred 
  • Must possess a valid state driver license and maintain automobile liability insurance as required by law
  • Must maintain dependable transportation in good working condition
  • Must be able to safely drive an automobile in all types of weather conditions
  • Must possess CPR certification for the healthcare provider
