*Job Summary*

The Hawaii Health Systems Corporation (HHSC) Kauai Region consists of two CAH hospitals and four ambulatory clinics. One of the hospitals is located in Waimea, Kauai. It is accredited by the Joint Commission and is licensed to operate a 45-bed facility. The other hospital is located in Kapaa Kauai. It is licensed to operate an 80-bed facility (9 acute psychiatric beds; 5 acute/swing beds; and a 66-bed dual certified skilled/intermediate nursing care facility). Also part of the Kauai Region is West Kauai Clinic (WKC), comprised of five clinics located in Waimea, Eleele, Kalaheo, and Poipu and Kapaa.

The Social Worker provides social services to patients and families; makes home visits; participates in referral, transfer, and discharge conference; prepares necessary transfer and discharge documents; makes financial studies (means tests) to determine ability to pay; processes the necessary documents to determine eligibility for admission into the Intermediate Care Facility; and maintains liaison with the Department of Human Services and other governmental agencies.

The Social Worker is responsible for providing crisis intervention, psychotherapy, family therapy, and comprehensive case management services for assigned patients/residents and their families. This position may also lead or assist with the referral and admission process for assigned new referrals and provides technical guidance to doctors, other hospital staff, and outside agencies on the methods and procedures for handling a particular case, within State and Federal guidelines. Position requires travel to and from other facilities within the region.

Requirements:

Satisfactory completion of a four-year course leading to a bachelor’s degree in social work form an accredited university or bachelor’s degree with a minimum of 12 semester credit hours in such courses as psychology, sociology, social welfare, social work or other related social science. Master’s degree from a social work program accredited by the Council on Social Work Education is preferred.

Applicants must have a minimum of one year of progressively responsible professional social work experience. Or successful completion of two years of graduate studies in an accredited school of social work plus one-half year of professional social work experience. Such experience must have demonstrated ability to perform the most difficult and complex assignments. One (1) year experience in a healthcare setting preferred.'

After receiving a conditional offer of employment, all prospective new HHSC employees are required to be fully vaccinated against COVID-19. New employees are required to provide confirmation of their full vaccination status by presenting their CDC vaccination card or VAMS Certificate prior to their start date.
