*Key Responsibilities:*
* Conduct assessments to understand clients' needs, strengths, and challenges.
* Develop and implement individualized service plans to address clients' needs and goals.
* Provide counseling and emotional support to clients experiencing crisis or distress.
* Connect clients with community resources and services, such as housing, healthcare, and financial assistance.
* Advocate for clients' rights and needs within the community and with other service providers.
* Maintain accurate and timely documentation of client interactions and progress in compliance with organizational policies and regulatory requirements.
* Collaborate with other team members and service providers to ensure coordinated and comprehensive care.
* Participate in regular supervision and professional development activities to enhance skills and knowledge.

*Requirements:*
* Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) in NY, or eligibility for licensure.

* Compassionate and empathetic approach to client care.
* Ability to work independently and as part of a team.
