*QUALIFICATIONS & EXPERIENCE REQUIREMENTS:*

* LPN in the state of Ohio
* At least two years experience as a nurse in long term care (not as an agency nurse).
* At least two years experience working as a nurse in long term care (not as an agency nurse)
* Must possess, as a minimum, a Nursing degree from an accredited college or university, or be a graduate of an approved LPN program.
* Must maintain current CPR certification
* A minimum of 5 years' experience in long term care.
