  • FMOLHS Core Competencies - Including HIPAA, Patient Bill of Rights, Safety Orientation, etc.; renewed annually
  • FMOLHS MS Fingerprinting - State requirement for Mississippi
  • MMR - Proof of immunizations or Declination accepted
  • Fit Test - Must be completed within 1 year prior to start, updated annually
  • Hepatitis B - Proof of immunizations or Declination accepted
  • Influenza Vaccine - Required during specific months or with appropriate exemptions
  • TB Test - Negative result requirements or follow-ups if positive
  • Health Statement - Signed and dated within 1 year of start stating ability to work without restrictions
  • Varicella - Proof of immunizations or Declination accepted
  • Tdap - Proof of vaccination within past 10 years or Declination accepted
  • Annual TB Questionnaire - Required within 6 months of start
  • FMOLHS Drug Screen - Negative 10 Panel within 30 days prior to start
  • FMOLHS Eye Exam & Color Vision - Ishihara Color Test within 1 year prior to start
