Schedule: Full time, 40 hrs/week, day shift, varied hours Monday and Wednesday 9 am - 6 pm, Tuesday and Thursday 7:30 am - 4:30 pm, Friday 7 am - 1 pm.

Responsibilities

Serve as a patient navigator to accommodate patients throughout the continuum of care.

Coordinate and lead presentations to prospective patients which may include community seminars, expositions and support groups.
Provide support with navigating the healthcare system and assist patients with utilizing available community support services.
Address barriers to care. Work with Navigation Team as needed and serve on hospital committees as assigned.
Work with physicians and the health care team to accommodate patient issues and to ensure quality patient care.

Requirements

Licensure / Certification / Registration:

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
Licensed Practical/Vocational Nurse credentialed from the Kansas Board of Nursing required.

Education:

High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.
