Job Overview:
As a Physical Therapist at CommuniCare Health Services, you will play a vital role in enhancing the lives of our patients through your expertise in physical therapy. You will work in a collaborative environment that values compassion, excellence, and teamwork.

Responsibilities:
- Provide specialized physical therapy services to patients in a LTC/SNF.
- Develop personalized treatment plans to address patients' specific needs and goals.
- Conduct thorough assessments to determine the most effective therapeutic interventions.
- Collaborate with healthcare teams to ensure coordinated and holistic patient care.
- Educate patients and their families on therapeutic exercises and techniques for continued progress.
- Maintain accurate patient records and documentation in compliance with regulatory standards.

Qualifications:
- Degree in Physical Therapy from an accredited program.
- Active state licensure as a Physical Therapist.
- Experience or interest in working with pediatric or acute care patients.
- Strong anatomy knowledge and understanding of hospital medicine practices.
- Ability to provide quality patient care with empathy and professionalism.
- Previous experience in outpatient or inpatient physical therapy settings is advantageous.
