What the position looks like:
As a Home Health Nurse you will perform home visits to assess the patients health problems and needs as well as develop and implement a nursing care plan. You will be responsible for delivering quality medical care in compliance with industry/company regulations, policies and procedures. You will also be responsible for active case management of your case load.

What your responsibilities will be:

Maintaining accurate, detailed reports and records using computerized documentation
Administering medications to patients and monitoring patients for reactions or side effects
Modifying patient treatment plans as indicated by patients' responses and conditions
Monitoring all aspects of patient care, including diet and physical activity
Instructing patients and families on topics such as health education or disease prevention
Be able to work in a fast paced environment

What you will need:

EDUCATION
The minimum level of education desired for candidates in this position is an Associate's degree. Advanced certification or willingness to earn advanced certification preferred.

POSITION QUALIFICATIONS
1. Graduate of an accredited practical nurse or vocational nursing program.
2. Has three (3) years nursing experience. Community/home health or medical/surgical experience is preferred.
3. Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
