Job Duties:

\* Receives report of resident’s condition from off-going shift and gives report to on- coming shift. Prepares a written report of pertinent information concerning residents on unit every shift.

* Monitors narcotics and keys at the beginning and end of each shift. Does not share narcotic keys with other nursing staff.
* Checks time schedule and takes an account of all scheduled staff members present, absent or tardy; fills out appropriate slip and turns into the Director of Nursing.
* Communicates verbally, supervises and delegates to the nursing assistant staff, resident assignments, giving such information as resident’s present condition or changes, daily regime of care to be delivered, special areas of concern for the individual resident and resident care goals.
* On admissions you are to do a head-to-toe assessment. Complete all documentation, transcribes medications to MARS and second checks with another nurse to prevent errors.
* Administers medications as prescribed, documenting them on MARS and noting any reactions; returns medication to pharmacy when discontinued.
* Document all PRN and any refused medications administer on the back of the MARS
* Monitors use of the emergency drug kit to ensure replacement and proper charge is made
* Makes rounds Q2 hours to observe and determine resident needs and assess adequacy of resident care. Observes and reports symptoms and condition of residents.
* Administers to special needs of the resident, i.e., decubitus care, isolation, tract care, suctioning, inserts IV’s, changes dressings, checks blood pressure and pulse as indicated.
* Records physician’s orders on physician order sheet and completes telephone orders as necessary
* Transcribes all medications and treatments prescribed by the physician to the MARS and communicates all changes to the appropriate personnel. Orders new medications from pharmacy when necessary.
* Notifies responsible party with any changes in resident’s status and all medication changes.
* After assessing the resident’s needs, in an emergency or immediate need, calls the physician to give report and receives further orders. In all other situations, assessing the residents needs and notifies the Director of Nursing when here or Charge Nurse for further instructions.
* Assists physician with treatments and physical exams as needed
* Maintains neat and accurate records reflecting resident’s condition, medication and treatment. Completes all documentation accurately and precisely.
* Demonstrates awareness that admissions, discharges and transfers are being properly carried out by the person assigned to that particular unit and all forms are completed correctly for each procedure. Concerning discharges, makes sure medical records are closed out properly, including the writing of a discharge summary.
* Gives direct resident care, when necessary, i.e., bathing, feeding, turning repositioning and providing incontinent care when needed.
* Maintains a good working relationship with personnel of unit, families, residents and other nursing home staff
* Keeps medicine carts and medicine room neat and clean
* Counsels with nursing assistants concerning work performance, infractions of rules, give “Notice of Recognition” or “Correction or Counseling” forms when deserved or needed.
* Attends Individual care plan conference when asked, and attends all staff and in-service meetings.
* Monitors and charts blood pressures, pulses and weights.
* Should work in harmony with all nurses.
