This is an hourly paid position (not paid by patient), with reimbursement for mileage, and the opportunity to be flexible with your schedule. If you've never worked in Home Health, we will provide you with a mentor as you start your role, and you will not be on your own until you are comfortable. This is a great opportunity for new graduates

This position will cover Medford and Stanley, WI

Responsibilities:

Evaluate patients' medical history and perform physical examinations to determine the best course of treatment
Develop personalized treatment plans and goals for each patient
Administer therapeutic exercises and modalities to improve patients' strength, mobility, and function
Provide manual therapy techniques such as trigger point therapy to alleviate pain and improve range of motion
Educate patients and their families on proper techniques for exercises and activities to continue progress outside of therapy sessions
Monitor patients' progress and adjust treatment plans as necessary
Collaborate with other healthcare professionals to ensure comprehensive patient care
Maintain accurate and up-to-date patient records

Hours:

Full Time or 1.0 FTE, 80 hours every 2 weeks.
Day shift, Monday - Friday and every 7th Saturday.
Flexibility in scheduling

Requirements:

Bachelor’s Degree in Physical Therapy from a school of Physical Therapy accredited by the American Physical Therapy Association (APTA).
Current Wisconsin and/or Michigan licensure to practice Physical Therapy required.
Current BLS certification or obtained within 90 days of hire
Ability to provide excellent patient care with compassion and empathy
Strong communication and interpersonal skills to effectively collaborate with patients, families, and other healthcare professionals
