*Your Role and Responsibilities*
As a Behavioral Health Clinical Supervisor, you will:
* *Lead Patient Care Delivery:* Plan, direct, and oversee the delivery of home health services, ensuring that each patient receives diagnosis-specific care tailored to their unique needs.
* *Coordinate Care Teams:* Oversee patient care services and personnel, ensuring smooth coordination between providers, vendors, facilities, and family members.
* *Maintain High Standards:* Ensure timely patient care starts, adhere to regulatory guidelines, and manage communications between all care team members.
* *Develop and Review Care Plans:* Approve and evaluate individualized patient care plans, integrating orders from all relevant physicians, and ensuring clinical appropriateness.
* *Monitor Quality of Care:* Continuously assess patient needs and adjust care plans to ensure optimal outcomes.
*What is Required?*
* Associates Degree in Nursing related field is required.
* 2+ years home care experience as a Registered Nurse in Home Health or Behavioral Health
* 1+ year supervisory experience
* Current state license as a Registered Nurse
* Proficiency with enterprise applications such as Workday and HomeCare HomeBase
* Proficiency with MS Office Suite (Word, Excel, Outlook)
