Responsibilities:
Conducting physical therapy assessments and developing individualized treatment plans
Providing physical therapy interventions to patients in a home-based setting
Documenting patient progress, treatment, and outcomes
Collaborating with other healthcare professionals and caregivers to ensure comprehensive care
Compensation and Benefits:
Pay up to $85.00 per regular visit
Start of cares and reevaluations are paid at a higher rate
Considered full-time therapists at 25 visits/week
Benefit packages including 401k match, paid holidays, short wait period for health benefits, and much more
Relocation Stipend Assistance
Minimum Requirements:
Graduate of an accredited Physical Therapy program
Current physical therapy license issued by the state of Colorado and a current CPR certificate
