Client in SD seeking OT-Contract (OT) to work Hours Per Shift: 08, Shift Start Time: 08:00 AM, Shift Notes: Facility name: Wakonda Heritage Manor, Centerville Care and Rehab, Sunrise Villa, Sunset Manor, Sunshine Terrace REQUIREMENTS: SNF exp, SD license. OT will be required to cover 5 facilities in the area, not all in 1 week but facilities can be shifted depending upon the needs across all 5 buildings. Productivity Requirement: Registered Therapists -83 +% ; Assistants - 90%CANCEL POLICY: 2 weeks # of WEEKS: 13SHIFT/HOURS:8/D PAYS GWW: YES - GWW 30 ON-CALL/HRS: n/a LICENSE: Yes - SD
