_We are seeking a compassionate and dedicated Long-Term Care Registered Nurse (LTC RN) to join our healthcare team. The LTC RN will provide high-quality nursing care to residents in our long-term care facility, ensuring their physical, emotional, and psychological needs are met. This role involves close collaboration with a multidisciplinary team to deliver comprehensive and individualized care plans that promote residents' well-being and quality of life._
*Key Responsibilities:*
* *Resident Care:* Provide direct nursing care to residents, including administering medications, monitoring vital signs, wound care, and assisting with daily activities such as bathing, dressing, and feeding.
* *Care Planning:* Develop, implement, and regularly update individualized care plans for residents based on assessments, doctors’ orders, and residents’ needs.
* *Assessment:* Conduct initial and ongoing assessments of residents' health conditions, documenting findings and communicating changes in residents’ conditions to physicians, family members, and other healthcare professionals.
* *Collaboration:* Work closely with a multidisciplinary team, including physicians, therapists, dietitians, and social workers, to coordinate comprehensive care for residents.
* *Documentation:* Accurately document all nursing care provided, including resident assessments, treatments, medications administered, and changes in condition, in accordance with facility policies and state/federal regulations.
* *Education:* Educate residents and their families about health conditions, medications, and treatments, ensuring they understand the care being provided and how to manage health conditions.
* *Compliance:* Ensure compliance with all healthcare regulations, including infection control, safety protocols, and facility policies and procedures.
* *Emergency Response:* Respond promptly and effectively to emergencies, providing appropriate nursing interventions and reporting incidents according to facility guidelines.
* *Quality Improvement:* Participate in quality improvement initiatives, staff meetings, and continuing education programs to maintain nursing competencies and stay current with best practices in long-term care.
*Qualifications:*
* *Education:* Associate's or Bachelor's degree in Nursing from an accredited nursing program.
* *Licensure:* Current and valid Registered Nurse (RN) license in the state of Delaware or compact license.
* *Experience:* Previous experience in a long-term care setting preferred but not required. New graduates are encouraged to apply.
* *Certifications:* BLS (Basic Life Support) certification required. Additional certifications in geriatric care or related fields are a plus.
* *Skills:* Excellent clinical skills, strong attention to detail, compassionate nature, effective communication skills, and the ability to work collaboratively in a team environment.
* *Physical Requirements:* Ability to lift, push, and pull up to [weight limit], stand and walk for extended periods, and perform physical tasks such as bending and kneeling.

3x12 Hr. Days (36 Hrs. per Week) _(Position is Monday, Tuesday, Saturday, Sunday, Thursday, Friday Rotating)_

$45.00/Hr. Blended Rate
