Job Title: Long Term Care Coordinator 1

Job Location: Arlington, FL

Job Type: Contract - 13 to 26 weeks

Job Duties:
* Managing a case load for healthcare members with long term care needs.
* Member assessments and notes.
* Complete assessments with members, caregivers, or providers to obtain information regarding client status, support system, and need for services for care plan development.
* Monitor delivery of services and follow-up with members, caregivers, or providers through in-person visits and telephonic contact
* Authorize and coordinate referrals for services.
* Ensure provider services are delivered without gaps and identify functional deficiencies in plans of care.

Job Requirements:
* 2+ years of Care Management experience- Required.
* Bachelor’s degree or Registered Nurse License and 2+ years of care management experience;
* Home health, discharge planning, or long-term care experience preferred.
* Bilingual in Spanish - preferred.
* At least basic Microsoft office experience
* Monthly and quarterly member contact and will include 80% travel. Remote role. Will require a driver’s license.

Schedule:
* 8 AM - 5 PM EST
* Mon-Fri
* 5x8

Note: Remote/ The employee will be mostly remote but have some member visit requirements.
