*Shift Information: *PRN; 8:00AM - 8:00PM
*Location: *Gloucester County, New Jersey
*Job Description:*
The *Home Health Aide* provides personal care and related services in the home. The aide functions under the direction, instruction and supervision of the staff nurse and the Director of Nursing and/or appropriate supervisor.
*Responsibilities:*
* Assist with ADLS and ADLs as per the POC
* Follow the POC for each client and document the same
* Help client maintain good personal hygiene
* Plan and prepare nutritious meals. Markets when indicated on the POC
* Assist the client with ambulation as per the POC
* Assists the therapy personnel as needed with rehabilitative processes
* May cue for medications ordinarily self-administered as assigned
* Taking and charting vital signs as per the care plan
* Assist with assigned treatments as ordered by the physician and approved and supervised by the nurse
* Prepares a report of visit on the day it is performed and incorporates same in the clinical record weekly or as directed
* Reports any change in the client’s status or home situation to her immediate supervisor, the staff nurse, or the aide supervisor
* Carries out their assignment as instructed by the nurse or the paramedical team and documents and reports to the nurse when they're unable to do so
* Performs routine housekeeping tasks as related to a safe and comfortable environment for the client, as instructed by the professional nurse
* Attends in-service as required by state and federal regulations
* Confirming weekly, the scheduling of visits with the Supervisor/Director, to coordinate necessary visits with other personnel.
*Qualifications:*
* Prefer 1 year experience as a CHHA in a health-related setting.
* NJ state certified CHHA course training
* Active CHHA certification required
* Transportation to assigned client visits.
