HOME HOSPICE LPN JOB REQUIREMENTS: It is NOT required that the LPN have Home Health experience, however we will consider Licensed Practical Nurses who have at least 1 year of recent experience working with adult and geriatric medical / surgical patients in any of the following settings: Rehabilitation Center / skilled nursing facility SNF, home health care / visiting nurse setting, rehab hospital, correctional facility, hospital / medical center, Intensive Care Unit ICU, Medical Intensive Care Unit MICU, Surgical Intensive Care Unit SICU, Definitive Observation Unit DOU, Critical Care Unit CCU, float / pool nurse. The licensed practical nurse must have been trained as a licensed practical nurse. The licensed practical nurse must have a current LPN license in the state. The licensed practical nurse must be willing and able to work Full Time hours. The Licensed Practical Nurse must either be a US Citizen or have your green card.

* 1. This is a full time position, the nurse can pick to work 5x8's, 4x10's or 3x12's. Are you willing and able to work full-time hours and do you have a preferred schedule?
* 2. The LPN can pick to work any day of the week. What days of the week (Sunday through Saturday) are you available to work?
* 3. We are ideally needing coverage in the afternoons, evenings and overnight but the hours are extremely flexible. What hours of the day/night are you available to work (The Licensed Practical Nurse will work with the other Home Hospice LPN's on the team to create and build a schedule that works for the whole team)?
* 4. This is not shift work, but is seeing multiple pt./day in their homes or senior living facilities. You will be driving from patient home to patient home & most patients will be close together but sometimes you may have to drive 20-45 minutes to reach a patient's home, therefore; about half your day will be spent driving. Traveling is part of your salary plus you will get additional pay for mileage, but I want to make sure that you are fine with traveling as that is an integral part of home hospice?
