*Job Description:* The LPN participates in the day-to-day coordination and delivery of skilled nursing services to children with specialized healthcare needs in their own home.

*Duties and Responsibilities:*
* Develops and maintains on-going relationships with management, department staff, case workers, patients, and family members in order to best meet the needs of clients.
* Ensures that delivery of nursing services is compliant with company/state policies.
* Available resource person to other health team members in the identification and fulfillment of patient needs.
* Maintains liaison with the community’s health-related resources.

*Qualifications, Knowledge, Skills, and Abilities:*
* Graduate of approved School of Professional Nursing - or -
* Current state Licensed Practical Nurse or Registered Nurse license
* Pediatric nursing experience preferred.

Job Types: Full-time, Part-time

Pay: $25.00 - $29.00 per hour
