Interim HealthCare is a New Hospice Agency operating in Ridgecrest, CA looking for a Per Diem Registered Nurse to join a family-oriented company, owned and operated by Saint Andrew's Abbey. We are an upcoming hospice company looking for a dedicated Hospice RN case manager to join our family-oriented team who is empathetic, hardworking, compassionate, and a dedicated team player. Our philosophy is recognizing all people are created in the image of Christ and we are committed to serving the dying and their families by offering love, compassion, and dignity on the journey home to God. We offer competitive hourly pay plus mileage reimbursement. Pay for patient care/visit hours, and pay for hours spent charting $45 - $55 per hour depending on experience. Essential Functions: * Performs assessments and procedures, which require substantial specialized knowledge, judgment, and nursing skill based upon principles of psychological, biological, physical and social sciences. * Visits in home clients to evaluate the quality of care being provided and reviews clinical notes. * Completes nursing initial and ongoing assessments of all in home clients within specified deadlines. * Participates in ongoing interdisciplinary assessment of the patient. * Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s). * Obtains physicians plans of treatment and orders, and initiates their execution. * Provides services that are ordered by the physician as indicated in the plan of care. * Counsels and educates the patient/client and family in meeting healthcare related needs. * Effectively communicates with other members of the interdisciplinary healthcare team providing care to promote coordination of patient/client care and planning for discharge. * Secures equipment and supplies and coordinates placement for use in the home. * Participates in Quality Assessment and Performance Improvement (QAPI) program. * Participates in in-services or training, as requested. Minimum Education & Experience Requirements: * Graduate of an accredited school of nursing. * Licensed to practice as a RN in California. License is active and in good standing. * 1 year of accumulated experience as a registered nurse within the last five (5) years. * One (1) year of hospice care experience preferred, but not required: willing to do admissions, continuous care, supervisory visits, PRN and routine visits. * Able to provide proof of valid driver's license if applicable. * Able to provide proof of valid auto liability insurance if assignment(s) include driving vehicle.
