Licensed Practical Nurse Primary Care Plaza

We are hiring a full time Licensed Practical Nurse for our Primary Care - Perryman Clinic at the Plaza.
The Schedule:
This is a Monday through Friday day shift position.
The Work:
LPN will assist with patient phone calls, procedures, rooming, and various administrative tasks as needed (ex: prior auths, RX refills, etc).
You will be working with a variety of providers and their patients, working in a blended role.
Provides phone triage, assisting with rooming patients, downloading blood glucose meters and pumps, and educating patients on injectable medicines and glucose meters.
Because you will not be assigned to one provider, the ideal candidate will be flexible and able to adapt to different provider preferences.
We are looking for a strong team player who is wiling to jump in and help when they see a need.
Previous experience with blood glucose meters and pumps is preferred but not required.
Provides support for assigned providers.
Completes patient care, patient education, prior authorizations, in basket messages,, prescription refills, and other duties as assigned.
Responsible for telephone triage, patient flow, and coordination of patient care
Schedules emergent need patient appointments
Identifies learning needs for patients and families, provides advice and recommendations, and inserts intravenous access with medication administration.
Delegates tasks to clinical staff in accordance with demonstrated competencies and consideration to Scope of Practice
Enters admission orders as directed by provider
Manages expired samples.
May be responsible for covering Medical Assistant job duties or other office duties as needed
The ideal candidate will be highly organized, task oriented, resourceful, and possess strong communication skills.
