*Position Overview:* As a Registered Nurse (RN) Case Manager in our Hospice service line, you will provide compassionate, high-quality care to patients in their homes, helping them navigate the challenges of serious illness. You will work collaboratively with a multidisciplinary team to ensure that each patient receives personalized care that aligns with their unique needs and wishes. *Key Responsibilities:*
* Manage and coordinate care for a caseload of hospice patients, ensuring that care plans are developed, implemented, and updated according to patient needs.
* Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs.
* Develop and revise individualized care plans in collaboration with the patient, family, and interdisciplinary team.
* Provide direct nursing care, including symptom management, pain control, and emotional support, to hospice patients in their homes.
* Educate patients and families on the hospice philosophy, goals of care, and the dying process.
* Serve as a liaison between patients, families, and healthcare providers to facilitate communication and coordinate care.
* Monitor and document patients' progress, adjusting care plans as necessary to meet changing needs.
* Participate in interdisciplinary team meetings to discuss and evaluate patient care plans.
* Ensure compliance with all federal, state, and local regulations, as well as company policies and procedures.
* Provide on-call support as needed to ensure that patients receive timely care and support.

*Qualifications:*
* Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
* Current State Licensee as a Registered Nurse RN
* 1 year of experience in a clinical care setting
* Experience in hospice or home health environment is preferred
* Nursing skills as defined by accepted nursing standards
* Ability to handle multiple priorities, documentation requirements, and deadlines
* Reliable transportation to perform job tasks
