Canyon Creek Post Acute is located in Castro Valley, CA 94546. We are looking for a committed, caring, and reliable Physical Therapist, PT to join our in-house rehab team for our Skilled Nursing Facility. As a physical therapist, you will have the opportunity to provide personalized care to patients, assisting them in reaching their physical goals and promoting overall wellness.

You will work closely with a multidisciplinary team, including doctors, nurses, and occupational therapists, to create and implement individualized treatment plans. Your expertise in evaluating patients, developing exercise programs, and utilizing various therapeutic techniques will be crucial in aiding patients in their recovery journey. With a focus on rehabilitation and pain management, you will play a critical role in improving patients' quality of life.

Position Type: PRN or Full Time
Pay Range: $60 - $65 per hour
Department: Rehab / Therapy

Specific duties include, but are not limited to:

Conduct assessments and develop individualized treatment plans for patients based on their specific needs and goals
Provide hands-on therapy, including therapeutic exercises, manual therapy techniques, and modalities
Monitor and assess patient progress, adjusting treatment plans as necessary
Educate patients and their families on exercises and techniques to continue progress outside of therapy sessions
Collaborate with other healthcare professionals to coordinate patient care and optimize treatment outcomes
Keep accurate and up-to-date patient records and documentation
Stay informed on the latest advancements and best practices in the field of physical therapy

Qualifications:

Graduate of an approved curriculum Physical Therapy
Is able to practice unemcumbered
Is in good standing with all regulatory agencies and licensing boards
Working knowledge of Medicare and other payer sources
Full knowledge or resident rights
Exudes professionalism in presentation
Ability to multitask
Must be able to read, write, speak, understand and communicate in the English language
CPR Certification
