Are you an experienced Home Health or Hospice RN? We have a need for a weekend Home Hospice RN for the Fairfield County CT area. The schedule is 24 hours, AM shift. We would welcome the chance to to speak with you about this opportunity.

  • Must be skilled in pain and symptom management.
  • Able to triage patient needs and make patient visits as appropriate.
  • Provide skilled and supportive nursing care to patients and families.
  • Perform assessment of patient and family, through observation, interviews, and analysis of records
  • Respond to physical, emotional psychosocial, environmental, needs.
  • Identify patient and family needs for other home health care services and makes referrals through agency-designated channels
  • Identify signs and symptoms of the disease and explains diagnosis and the nature of palliative care to the patient and family as appropriate
  • Utilize appropriate nursing interventions to alleviate pain and discomfort in accordance with the plan of care.
  • Supports decision making by the patient and primary caregivers
  • Minimum of two years of nursing experience, preferably in medical/surgical nursing required Valid Clean Driver’s License Required Home Health Care and/or Hospice / End of life care experience preferred Active client contact within past three years, preferred. Certified Hospice and Palliative Care Nurse (CHPN) preferred
