Physical Therapist: Home Hospice

We are currently seeking an experienced Physical Therapist to provide direct care to meet the social, physical, recreational, emotional, and spiritual needs of our residents in our Skilled Nursing Home.
Position Summary
The primary purpose of your position is to provide direct physical therapy to meet patients’, residents and family needs, ensuring compliance with hospice and palliative care regulatory and Joint Commission standards and with Chandler Hall standards.
Assist in planning, developing, organizing, implementing, evaluating, and providing the physical therapy programs of the hospice.
Essential Functions
Plan, develop, organize, implement, evaluate, and direct physical therapy, as well as its programs and activities, in accordance with current rules, regulations, and guidelines that govern the Hospice Department
Upon physician’s order for P.T. consult, reviews patient’s medical records and performs physical assessment to determine physical therapy required
Identifies patient goals of care and limitations to meeting goals.
Assess, identify and teach caregivers safe care of patients with life limiting illness.
Assist in standardizing the methods in which physical therapy will be accomplished.
Performs muscle tests, range of motion measurements, gait, and functional analysis and records them to aid in establishing P.T. treatment and goals.
Instructs patient and/or patient’s family in P.T. procedures to be continued at home.
Provides written initial evaluation with recommendations for P.T. treatment based on the physical assessment that will make easy processing of orders by the physician.
Maintain a reference library of written therapy material, laws, etc., necessary for complying with current standards and regulations, and that will provide assistance in maintaining quality resident care.
Participate in discharge planning, development and implementation of client/resident care plans, client/resident assessments, etc., as necessary
Assist with Quality Assessment and Assurance in developing, implementing, and maintaining an ongoing quality assurance program for physical therapy service.
Maintain treatment records, resident files, and progress notes as required.
Perform administrative duties such as completing medical forms reports, evaluations, studies, charting, etc., as necessary.
Education and Experience
Graduate of an accredited program for Physical Therapy. Licensed in the State of Pennsylvania as a licensed physical therapist.
2 years of hospice/home care experience preferred.
