*Monday - Friday, Shift: 7:00 a.m. - 3:00 p.m. for a small CIRT program located in Belchertown, Mass. *

Three Rivers is a Clinically Intensive Residential Treatment Program (CIRT) serving children 6-12 years old and their families.

Our multi-dimensional modalities of treatment are deeply rooted in supporting the ability to experience and learn the fundamental skills that develop in relationships with others, including safety, belonging, self-regulation and respect.

During the time that a child resides at the program we work to help them and their family to grow in the ability to enjoy positive and helpful relationships, manage stress and challenging feelings, participate in school and daily routines, and to recognize and champion their own values and strengths.

The LPN will be responsible for the provision of a range of medically / nursing related services, the coordination of client health care. Performs a variety of duties as a member of the milieu staff team to provide a safe therapeutic and normalizing environment for the client population.

Essential Position Functions

1. Routine and specialized care of all clients - in direct collaboration with child psychiatrist, associated medical providers and nursing manager – providing routine first aid and urgent care to all clients, utilizing each client’s treatment plan and Three Rivers Program protocols.

2. Work as a professional member of the program’s treatment team, attending all specifically-assigned treatment meetings (attending all necessary “mini-team” meetings).

3. Assess medical health needs of clients. Establish health care objectives and interventions to meet goals specific to individual treatment plans. Accept responsibility for ensuring completion of documentation for incidental events. Communicate all pertinent information to all staff, families and practitioners while working directly with the nursing manager, milieu supervisor, advocates / alternates and clinicians. Prepare daily shift reports.

4. Maintain medication administration system according to established regulations and procedures. Optimal maintenance of clients’ medications / medical supplies. Maintain supply of gloves and eyewash on premises.

5. Manage employee / client injuries and exposures adhering to Three Rivers Program Infection Control Plan and required first aid care. Provide follow-up of critical incidents and necessary employee counseling for such. Manage removal of biohazard waste.

6. Coordinate various health care teaching and group work for clients in collaboration with nursing manager and milieu supervisor.

7. Monitor clients’ nutritional status and work with the program’s dietician and nursing manager to develop to develop individual dietary plans

8. Adhere to Standards and Ethics policy.

Minimum Requirements for the Position

Degree: LPN Licensed in Massachusetts

Years and Type of experience: 2 years related experience

Other skills or requirements: Good verbal and written communication skills, excellent interpersonal skills, valid driver’s license.

Physical requirements: Visual and auditory acuity, and manual dexterity and coordination to perform nursing duties.
