We are seeking a patient focused, compassionate Nurse Practitioner or Physician Assistant to join our interdisciplinary team in Palliative Care
Position Details:
- Full-time, Day Shift
- No call, no weekends
- Outpatient - visit patients in their homes
- Key Responsibilities include seeing patients, leading family meetings, writing orders and notes. Serve those most in need by managing symptoms and determine goals for care.
- Patient Volume: 4-5 new patients per week, as well as follow-up visits with existing patients
Qualifications:
- Master's Degree required
- ACNP, AGNP, FNP or PA Certification required
- Candidate must have experience in hospice or a setting with high mortality
- If new grad, minimum 5 years' experience as an RN
- ICU, Oncology, Palliative Care, or Hospice Experience
- If new grad, minimum 5 years' experience as an RN
- Ideal candidate is compassionate, collaborative, and has strong communication skills
