As a Nurse Practitioner in Internal Medicine at SSM Health St. Mary's Hospital in St. Louis, Missouri, you will play a key role in delivering high-quality primary care services to adult patients within an outpatient setting. You will collaborate with a multidisciplinary team to assess, diagnose, and manage a variety of acute and chronic medical conditions.

Key Responsibilities:

  • Patient Assessment and Diagnosis: Conduct thorough health assessments, including medical history reviews, physical examinations, and diagnostic tests to evaluate patients' health status and formulate accurate diagnoses.
  • Primary Care Services: Provide comprehensive primary care services to adult patients, focusing on health promotion, disease prevention, and management of chronic illnesses such as diabetes, hypertension, cardiovascular diseases, and respiratory conditions.
  • Treatment Planning and Management: Develop and implement individualized treatment plans based on evidence-based practices, including prescribing medications, ordering and interpreting diagnostic tests, and coordinating referrals to specialists as needed.
  • Patient Education and Counseling: Educate patients and families about health conditions, treatment options, preventive care, and lifestyle modifications. Empower patients to actively participate in their healthcare decisions and self-management strategies.
  • Collaborative Care: Collaborate with physicians, nurses, pharmacists, and other healthcare professionals to ensure coordinated and integrated patient care. Participate in interdisciplinary rounds, case conferences, and care coordination meetings to optimize patient outcomes.
  • Documentation and Record-Keeping: Maintain accurate and timely electronic health records (EHR) documentation of patient encounters, treatment plans, and follow-up care in compliance with regulatory standards and organizational policies.
  • Quality Improvement Initiatives: Participate in quality improvement projects and initiatives to enhance the quality and safety of patient care, promote clinical excellence, and achieve organizational goals.

Qualifications:

  • Master's degree in Nursing from an accredited program
  • Current licensure as a Nurse Practitioner in Missouri with national certification as an Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) or Family Nurse Practitioner (FNP)
  • Minimum of 2 years of experience practicing as a Nurse Practitioner in internal medicine or a related specialty preferred
  • Strong clinical assessment, diagnostic, and therapeutic skills
  • Excellent interpersonal and communication skills
  • Ability to work independently and collaboratively within a multidisciplinary healthcare team
