Home Health Care RN- wellness nurse for a TalentBurst Private client.
- Provide warm and engaging personal care services, assistance, support, and companionship.
- Administer/monitor medication for the patient based on the prescribed care plan.
- Engage with the patient while checking on their well-being and communicate changes in condition or care plan.
- Monitor Blood Pressure and assist with mobile movement and meal plans.
- Collaborate with the patient's family and health care team in assessing and documenting resident assessments at set intervals.
Must be BLS/ACLS certified, NIH (Stroke certification), Licensed Practical Nurse (LPN/LVN) or Registered Nurse preferred.
- Client requested a visit 3 times a day, 8 am-Noon-5 pm to start.
- 12-week assignment to start.
- Location: Fort Collins CO.
