*What will you be doing:*

The _Intake Coordinator_ coordinates activities of order intake, evaluation of admission criteria, referral/authorization of cases, and transition of care for the Home Health Agency; staffs initial nurse, contacts patient/family member, and tracking of referrals. The intake coordinator acts as liaison to case management ensuring patient care.

*What will you come with:*
* Minimum of some college coursework
* Minimum of one (1) year of nursing experience, in home healthcare preferred
* Active Practical Nurse (LPN) license in the State of Florida.

Specialized knowledge
* Knowledge of the Home Health Services
* Knowledge of referral/authorization process
* Knowledge of computer clinical programs such EHR/EMR system(s)

Skills & Abilities
* Great communication skills, both written and spoken.
* Bi-lingual preferred
* Strong organization skills and be detail oriented
* Ability to maintain confidentiality of sensitive information.
