The Registered Nurse RN Case Manager will report to the Director of Patient Care Services and will join a leading medical facility that stands as a cornerstone of healthcare excellence. As a Registered Nurse RN Case Manager, the individual assumes a vital role in coordinating and optimizing patient care, ensuring seamless transitions throughout the healthcare continuum. Responsibilities include conducting thorough assessments, collaborating with multidisciplinary teams, and developing personalized care plans to enhance patient outcomes while advocating for efficient resource utilization and maintaining a strong focus on patient satisfaction and well-being.
