*JOB DESCRIPTION:*
LPN provides long-term and rehabilitation care to residents providing routine daily nursing care according to the resident’s care plan. .

*RESPONSIBILITIES:*
* Provide direct care and treatment to long-term and rehabilitation residents
* Provide assessment and documentation of residents under their care
* Communicates with the Nursing team and Medical team as appropriate
* Maintains competency in all site-specific tasks required by the facility

*REQUIREMENTS:*
* New York State LPN licensure
* Experience in long-term care facility preferred
* Able to perform tasks while upholding high standards of quality, cleanliness, and kindness
* Physical endurance for frequent activity; ability to lift a minimum of 20 pounds
