MSW or BSW Social Worker - Hospice - Part-Time

Schedule: Part-Time, Thursdays, 9am - 5pm (hours can be flexible)

Location: Can work from hospital or remote, visits for admissions would be within a 30 mile radius of the Olivia, MN Hospital or in that county. (3 major nursing homes in the area, some patients at home, some may be in the hospital).

Pay rate: Up to $72/hour Depending on Experience

*This is a temporary assignment filling in for a Leave of Absence, expected to last until early October 2024

What we ask of you:

  • Help manage services/care plan for a caseload of hospice patients.
  • Attend bi-weekly meeting with IDT/Interdisciplinary team to review patients with the Case Managers, RN, CNA and Director.
  • Complete admissions assessments as needed (approximately 5-6 per month)
  • Complete in-home, facility or hospital visits as needed to re-assess hospice patients needs.
  • Complete accurate and timely documentation of services provided.
  • Not currently any high risk bereavement patients at this time and don't expect to have any, but it is a remote possibility. 

 

Required Skills/Qualifications:

  • An active LSW license in the state of MN.
  • At least a BSW in Social Work, MSW is a plus.
  • At least one year of previous experience working as a LSW, preferably in a hospice setting. 
  • Experience completing Psychosocial Evaluations is required.
  • Must have a reliable workspace/internet at home and reliable transportation for client visits. (Site will provide a computer/phone)
