*Shifts -*

*3pm-11pm*

*Duties and Responsibilities for a LPN include but are not limited to the following:*

*LPN Licensed Practical Nurse* *Responsibilities:*

* Performs nursing care in relatively structured, routine situations. Administers multiple forms/methods of treatment, under the direction of the RN Supervisor
* *Prepares residents for diagnostic tests, treatments, examination, PT/OT, etc., according to protocols.Sets- up required documentation for physician visits and transcribes orders timely.*

* Prepares and administers medications, according to protocols.
* Follow facility procedures /protocols for admitting residents, transferring residents to hospitals/clinics/outpatient and completion of appropriate documentation based on established guidelines.
* Administers intravenous infusion in accordance with the facility’s Nursing Department Policies, procedures and Infection Control Guidelines.

*Educational Requirements*

Successful completion of State Approved Nursing Assistant Training and Competency Evaluation Program. Good standing in the state registry is required.
