Job: RN Case Manager

Location: Santa Rosa, CA 95403

Shift: Day 5x8-Hour (08:00 - 17:00)

Duration: 13 week

The Utilization Coordinator is responsible for coordinating admissions, performing concurrent and retrospective utilization review, and facilitating discharge planning. S/he also provides coordination of complex discharge planning needs and referrals to appropriate support services.

Requirements:
- CA RN Licensure
- 2 years experience working in Utilization Management (admissions, utilization review, and/or discharge planning)\*CCM-Certified Case Manager preferred\*. Equivalent experience will be accepted in lieu of the required degree or diploma.
- BLS (AHA)
