Patient Care:- Provide triage and primary care services to patients (ages 12 & up) through in-person and tele-health visits * Ability to triage patients based on severity of condition * Conduct services for patients with chronic medical conditions to improve quality patient care and reduce hospital readmissions and unnecessary emergency room visits * Interviews patients; takes medical histories; assesses patient health; performs physical examinations; administers injections/ immunizations; performs suturing, splinting, casting; manages wounds and infections- Instructs / counsels patients; explains treatments and medications; counsels / promotes emotional and physical health and wellness * Educate the patient on properly contacting the physician's office for urgent care matters / or 911 when appropriate or unsure * Work closely with the triage team to manage the day-to-day calls involving: symptom control, and medication management, and provide patient and family education.- Obtains / studies medical histories; documents patient and clinic records- Administers / orders diagnostic tests; interprets results; makes diagnoses; prescribes medications and other treatments- Identifies short- / long-range patient care issues; recommends courses of action; implements physician directives- Provides / ensures continuity of care; develops / implements patient care management plans- Complies / ensures compliance with federal, state, and local legal and professional requirements, and infection control protocols and policies- Develops health care team staff; provides training and educational / growth opportunities- Maintains professional / technical knowledge; reviews professional journals; establishes personal network; participates in educational workshops and professional associations- Collaborates with clinic staff to recommend / improve patient care practices, and clinic guidelines and workflow- Ensures adherence to HIPAA regulationsWhat is your background?
