Job Posting Summary Baptist MD Anderson is looking to add a Registered Nurse (RN) as a Nurse Navigator at the Baptist MD Anderson Clinic in Jacksonville, FL. This patient navigator will support our Malignant Hematology Department. This is a 40-hour position with shifts Monday-Friday from 8:00 AM -5:00 PM. Additional responsibilities for the RN Nurse Navigator may include: * Act as an educator and advocate for all newly diagnosed cancer patients and their families * Coordinate the care of hematology patients across the health care continuum, particularly Primary Care Social Workers/Care Coordinators * Promote and support opportunities for participation in clinical trials * Maintain follow-up with patients while at home to provide educational support and comfort to ensure recommendations are followed * Promote healthy lifestyle choices and self-care strategies through education and appropriate referrals to supportive services This RN Nurse Navigator will be located at Baptist MD Anderson in downtown Jacksonville, FL.
