Position Summary:

Manages the departments activities and resources to drive quality outcomes.

Reports to: Director

FLSA: Non-Exempt

ESSENTIAL FUNCTIONS:

-Review Electronic Medical Record (EMR) and payor registries to identify high risk patients.
-Analyze clinical data and identifies Hierarchical Condition Category (HCC) gaps.
-Query/communicate with provider/care manager to capture accurate Hierarchical Condition Category (HCC).
-Assist in the evaluation of data collected to assure accuracy and reliability. Prepares reports and performance reporting of quality data.
-Oversees quality reporting to the insurance carriers.


Minimum Qualifications:

Minimum Education
Associate's Degree in Nursing - Required
