Klap6 is currently looking for a RN Home Health to work onsite at our client's healthcare facility in Newton, MA.

*Shift: *07:00 - 17:00

*Summary*
* As a Home Health RN, responsibilities not limited to assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families.
* Patient care is delivered within organizational philosophy, policy and standards of community health practice.
* Also required to perform admission processes and case management responsibilities in accordance with MGB Home Care's policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.

*Requirements:*
* 1 year Traveler and Home Care experience.
* 1 year Case Management Experience.
* Ability to travel to cases as assigned.
* Currently BLS certified
* Current valid state RN licensure.

Job Type: Contract

Pay: $60.00 per hour

Expected hours: 36 – 40 per week
