Job Summary and QualificationsThe Intake Assessment Coordinator in conjunction with the Program Director coordinates the pre-admission assessment process and provides clinical assessments for potential patients. Also, assists with community education with clinical issues and participates with community events such as health fairs.
- You will complete assessments in a timely manner.
- You will interact and communicates with others in a way that promotes a positive and cooperative work environment.
- You will be able to assess and reassess needs of patients.
- You will provide on and off-site clinical assessments for potential patients to the program.
- You will work collaboratively with hospital departments to assist managing behavioral health assessment needs.
- Participates as an active member of the multidisciplinary team.
- You will provide ongoing education to patients and families as needed.
- You will provide resources and support for patients before, during and after treatment.
- Basic Cardiac Life Support must be obtained within 30 days of employment start date
- Licensed Family Therapist (LFT), or Licensed Clinical Social Worker (LCSW), or Registered Nurse
- Associate Degree, or Registered Nurse Diploma
- Nonviolent Crisis Interv Inst, or Nonviolent Crisis Intervention must be obtained within 30 days of employment start date
