Description

The RN navigator serves as a consistent resource to support optimal care delivery for patients and other projects as assigned, focusing on coordination of care during pre-procedure, hospitalization, discharge planning and facilitating post-hospitalization follow-up. The nurse navigator is knowledgeable of multiple treatment decisions, evidence-based standards, best practice guidelines and maintains care collaboratively across the continuum. In collaboration with clinicians, identifies and facilitates optimum care delivery with clinical expertise, strong interpersonal abilities and critical thinking skills. Provides effective oral or written feedback to providers to identify care gaps.

Required Qualifications:

  • Washington Registered Nurse License upon hire.
  • 1 year experience as a Registered Nurse in Nephrology services.

Preferred Qualifications:

  • Bachelor’s Degree in Nursing (BSN)

About the Team

Providence Physician Enterprise (PE) is a service line within Providence serving patients across seven states with quality, compassionate, coordinated care.
