AuthoraCare Collective is currently seeking a Full-time Speech Therapist (Monday - Friday 8:00am - 5:00pm) for our Integrated Health Services (Skilled home-based-care) department supporting our High Point, Kernersville and Winston-Salem service areas. Must be able to travel to, enter, and function in the homes of any patient within the service area.
The Speech Therapist provides speech or language therapy services in accordance with the patient's plan of care as directed by their primary care provider. Manage the speech therapy Plan of Treatment for a case load of Integrated Health patients in collaboration with other members of the interdisciplinary team (IDT). Assess and identify patient problems, establish therapy goals, and interventions. Work with physician to establish a treatment plan using a variety of methods, including exercises, hands-on therapy, and equipment to facilitate recovery. Establish and maintain therapeutic relationship with patient, including providing community support for resources and education.
Document the goals and outcomes of care. Therapist may perform evaluations and treatments for hospice patients as needed. Master's or doctoral degree in speech-language pathology.
Minimum of one-year speech therapy experience home health or facility preferred.
Current and unrestricted Speech Therapy Licensure.
