*Job Summary*
We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team. The ideal candidate will provide high-quality patient care in various settings, including outpatient clinics, hospitals, and home health environments. You will be responsible for assessing patient needs, developing individualized care plans, and implementing effective treatment strategies to enhance mobility and improve overall physical function.

*Responsibilities*
* Conduct thorough evaluations of patients to determine their physical therapy needs.
* Develop and implement personalized treatment plans based on patient assessments and goals.
* Provide hands-on therapy techniques to improve patients' mobility, strength, and overall function.
* Educate patients and their families about treatment options, exercises, and home care strategies.
* Collaborate with other healthcare professionals to ensure comprehensive patient care.
* Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.
* Participate in discharge planning to facilitate smooth transitions for patients returning home or moving to other care settings.
* Maintain accurate documentation of patient evaluations, treatment plans, and progress notes.

*Qualifications*
* A degree in Physical Therapy from an accredited program.
* Valid state licensure or eligibility for licensure as a Physical Therapist.
* Strong knowledge of anatomy and physiology relevant to physical therapy practices.
* Experience in outpatient settings is preferred; experience in pediatrics or inpatient care is a plus.
* Familiarity with discharge planning processes and home health services is beneficial.
* Excellent communication skills with a focus on patient care and education.
* Ability to work collaboratively within a multidisciplinary team environment.
* Commitment to ongoing professional development and staying current with industry best practices.

Pay: Up to $41.00 per hour
