Professional Growth Opportunities
Daily Pay (Work today. Physical Therapists are responsible for providing physical therapy services to patients in their own homes. The PT will assess patients' conditions, develop personalized treatment plans, and deliver therapy to help improve mobility, reduce pain, and restore function. The role involves traveling to patients' homes and adapting treatment plans to suit home environments while ensuring high-quality care and patient safety.
 
 
Assists in development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy.
Treats clients to relieve pain, develop or restore functional and maintain maximum performance, using physical means, such as exercise, massage, heat, light and electricity.
Teaches, supervises, and counsels the family and client in the physical therapy program. Instructs client in care and use of adaptive devices such as: wheelchairs, braces, crutches, canes and prosthetic and orthotic devices.
Records on a timely basis all evaluation data, treatments, and client's response to therapy intervention. Also records quality assurance data as directed.
Participates in case conferences, physical therapy staff meetings, agency activities and in-service programs as necessary. Participates with staff, clients, and physicians in discharge planning activities and completes a Physical Therapy Discharge Summary
Supervises the home health aide every 14 days when physical therapy is the only professional discipline involved in the client's care.
Evaluates the client, the client’s significant other(s) and the home situation to determine what instruction will be required, what assistance will be available from the family in caring for the client, and what other Agency and community services will be required
# Active participation as required per policies and procedures related to the Agency’s QAPI program.

 
# Awareness of appropriate reasons for a patient discharge and Agency’s policy on appropriate reasons for discharge.

 
# Awareness of infection prevention and control techniques, both within their scope of practice and within the Agency’s policy and procedures. Awareness and participation in the Agency’s infection control program.

 
# Appropriate bag technique policy acknowledgement and demonstrated competence when applicable. Physical therapy equipment and other medical equipment will be utilized. Hand washing materials and PPE.

 
 
Incumbents must be compliant with conditions of participation for Center for Medicare/ Medicaid Services.
 
Must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
· The Physical Therapist in charge of PT services must have a home care experience and at least one year of supervisory experience. (PHD or better
