· The home care nurse(RN)’s responsibilities include performing patient visits and monitoring their conditions, conducting tests, and writing reports for the physician. The ideal candidate will provide quality, skilled and/or palliative patient care to our home-bound clients, conduct home visits as assigned, assessing the patient’s condition, progress toward goals, providing patient care, teaching and other skilled interventions as ordered, while adhering to State Nurse Practice Act, agency policy, and accepted professional standards of practice, while providing nursing services utilizing a comprehensive base of nursing theory and nursing process, and communicates/documents observations and assessments.
*Traveling to patients’ homes and managing their care plans according to physicians’ instructions.
· Testing for muscle weakness, bedsores, and any signs of infection.
· Educating caregivers and family on the aftercare or ongoing care of the patient.
· Monitoring patient recovery and compiling reports for the physician.
· Graduate of an accredited school of professional nursing. Minimum of one (1) year experience in an acute care setting or equivalent experience.
* Homecare/Hospice experience preferred.

· Nursing skills as defined as generally accepted standards of practice
· Excellent report writing skills.
· Completion of an accredited Registered Nursing Program.
